"The Insider" has obtained the booking sheet for one of the two doctors who, along with Howard K. Stern, have been charged with felony conspiracy to furnish drugs to the late Anna Nicole Smith.

Sandeep Kapoor, 40, was arrested Thursday in L.A. at 6:33 p.m. and booked at 7:13 p.m. His bail amount is set for $20,000.
